Items Needed To Register Students

The following items are required for student registration:

State certified birth certificate
Social security card
Health record of required immunization,
Assignment letter, if applicable,
Custody papers, if applicable, AND

Three (3) Proofs of residence must be from the following list:

Current utility bill (gas, water, or electric),
The current year W-2 form,
The most recent federal or state income tax returns,
A vehicle insurance print-out from the parent’s or guardian’s insurance company,
A signed residential building contractor agreement,
A signed real estate lease agreement or signed mortgage loan documentation.
All documents must show the name of the parent or guardian, the utility service address, and the street or road address of residence.

Pre-Kindergarten Registration - Additional Requirements

In order to verify income to qualify for Pre-Kindergarten Registration, at least one of the following documents must be supplied.

Most recent Income Tax Form (1040 or 1040A)
W-2 Form (all forms from household)
Social Security Award (family total)
SSI
Child Support
Pay Stubs (2 most recent)
Proof of receiving Food Stamps
Approved Free or Reduced School Lunch
